Change of Name/Addition of Spouse’s name

CHANGE OF NAME/ADDITION OF SPOUSE’S NAME

“CHANGE OF NAME /ADDITION OF SPOUSE’S NAME IN PASSPORT ON RE-ISSUE BASIS”

I.Following marriage, remarriage or divorce :

a. An applicant applying for re-issue of Passport for incorporation of the name of his/her spouse,

b. A woman applying for change of name/surname in existing Passport due to marriage

c. Divorcees applying for change of name OR for deletion of spouse’s name in existing Passport,

d. Re-married applicants applying for change of name/spouse’s name in passport.

Except the details provided by the applicant (of any category mentioned above) in the application form Annexure E, no proof of marriage /dissolution of marriage (court’s order for judicial separation/.decree of divorce) etc. is required.”

II.In other circumstances for change of name, the applicant (both male and female) should furnish :

a. Clippings of two local newspapers or the Gazette notification of the concerned State Government, as the case may be,

b. At least two public/ government documents issued in the desired /applied changed name to ascertain that the applicant has actually changed his name.

Note: Fresh passport will be issued in all cases of name change. Please follow the procedure for Re-issue of passport and submit ONLINE form along with above supporting documents, if applicable (for case-I) along with Annexure E.

Note:

  1. The completed online application may be submitted at the Embassy counter on any working day between 0900 hrs to 1100 hoursand at the Consulates on any working day between 09.30 to 12.00 hours.
  2. Applicants may sometimes face difficulty in accessing the online application format due to technical reasons. In such circumstances, it is advisable to try after a while. Applicants are also advised to use computers with latest version software operating systems.
  3. Applicants may please note that New Passport booklets are printed in India (Delhi). Applicants are intimated through e-mail cons.beijing@mea.gov.incons1.beijing@mea.gov.in (from Embassy), cons.guagnzhou@mea.gov.in&cons2.guangzhou@mea.gov.in (from CGI Guangzhou) and  vc2.shanghai@mea.gov.in&vc4.shanghai@mea.gov.in (from CGI Shanghai) on arrival of their passports from India.
  4. Application has to come in person to submit the passport form.
